Look at the firebox. Metal panels with visible seams, a metal surround and a straight metal flue mean factory-built; brick or refractory panels with mortar joints and a masonry stack mean the other. The service is different enough that it is worth knowing before you book.

On a manufactured fireplace those are refractory panels and they are a replaceable part, which is better news than it sounds. A cracked panel is still a reason to stop using it until it is dealt with, because its job is keeping heat off what is behind it.

On a split-level that is very often a duct branch that has come off its takeoff inside a joist bay. The register still blows a little, so nobody suspects it, and the system spends every winter heating the space between two floors. Somebody has to follow the run to find it.
